The sacrum is composed of five fused vertebrae (S1–S5) that form a triangular bone at the base of the spine. These vertebrae unite between ages 18 and 30, creating a single, stable structure. The sacrum features a concave anterior surface and a convex posterior surface, with nerve outlets on the anterior and lateral sides. It connects to the hip bones via the sacroiliac joints and articulates with the coccyx at its inferior end. This fusion provides structural support for the pelvic girdle and protects the spinal cord's lower region.

  • Fused Vertebral Bodies: The main, solid part of the bone is formed by the fused bodies of the five sacral vertebrae, with transverse ridges on the anterior surface representing the fusion lines.
  • Sacral Canal: A continuation of the lumbar vertebral canal that runs down the center of the sacrum, containing the cauda equina (nerve roots) and filum terminale.
  • Sacral Hiatus: The inferior termination of the sacral canal, where the laminae of the S5 vertebra fail to fuse.
  • Sacral Foramina: Four pairs of anterior and posterior holes (foramina) that allow sacral nerves and blood vessels to pass through.
  • Alae (Wings): Large, fan-shaped lateral masses on the upper surface (S1) that connect to the pelvis.
  • Sacral Crests: The posterior surface features five vertical ridges:
    • Median Sacral Crest: Fused spinous processes.
    • Intermediate Crests: Fused articular processes.
    • Lateral Crests: Fused transverse processes.
